Fire Report / Incident Report
A fire report or incident report contains information regarding the Merced Fire Department (MFD) response to a specific call. Details included on these reports include, but are not limited to: response times, unit(s) responding, nature of the call, and/or actions taken.
The fastest method for obtaining individual fire reports or incident reports is to email the request to MFD Records. Please be sure to include your contact information, email and/or phone number, in case we need to clarify your request and the email or address to which you would like the report sent.

Pre-Hospital Care Report (PCR)
A MFD Pre-Hospital Care Report (PCR) contains information regarding the medical treatment of an individual patient by the Merced Fire Department.
The release of medical records is restricted by the terms of the Health Insurance Portability and Accountability Act (HIPAA). Individuals may request a copy of their own patient care record. These records may also be obtained by a parent, guardian, conservator, those who have medical decision-making authority for an individual, agencies having a patient-signed consent to release medical information, or by the executor of a decedent’s will.
The MFD Pre-Hospital Care Reports cannot be requested by phone as proof of identity is required to establish eligibility to receive these records under the Health Insurance Portability and Accountability Act (HIPAA).
